We appreciate your referral! We’re not in EPIC yet so w’ve provided some information below to make the referral process easier. Please give us a call at 619-317-1505 should you need additional information or have questions.

An authorization and referral with nutrition related diagnosis are always required before we see clients.

AUTHORIZATION:

When placing a prior authorization in EPIC, please ask for the following CPT codes and units:

97802 – 6 units

97803 –

150 units (for any eating disorder or feeding disorder diagnosis)

60 units for all other diagnoses

REFERRAL:

Please fax the referral to 858-924-0506 with a nutrition related diagnosis
